Tech solutions for end-to-end management
A HIPAA-and FERPA-compliant online application is used to quickly submit student referrals, match them with services, and track their progress.
Proven models and frameworks
The Student Health Hub is built on proven models and frameworks for delivering school-based services, like SBIRT, MTSS, and WSCC.
SBIRT
Screening, Brief Intervention, and Referral to Treatment.
Making it easier for schools to scale and sustain their SBIRT programs.
MTSS
Multi-Tiered Systems of Supports.
Helping schools coordinate services with community providers to ensure all students have access to the support they need to succeed.
WSCC
Whole School, Whole Community, Whole Child.
Helping schools, city governments, and their partners align resources in support of the whole child.
Student’s journey
Here’s an example of a student’s journey through SBIRT:
Screening
A student takes an interactive, youth-friendly survey to assess their strengths, challenges, and needs.
Brief intervention
Based on their responses, students may meet with the school counselor to dig a bit deeper and talk about next steps.
Referral to services
Students who need additional help are referred to mental health and substance use providers who are part of the Student Health Hub.
Additional support services
Students can also be referred to help with food, housing, transportation, and other needs to help support their healthcare goals.
